Clients managing a diagnosed condition are encouraged to keep working with their physician and to review any herbal plan with them.

Booking an appointment or submitting a healing plan request confirms that you have read, understood and agreed to the terms below.
Not a doctor, not medical care
The provider at TLC Self Care is a Connecticut licensed aesthetician and certified herbalist. She is not a physician, surgeon, nurse, physical therapist or licensed medical professional. Nothing offered here is medical treatment, medical advice, diagnosis, or a substitute for care from your doctor or surgeon.
No guarantees of results
Aesthetic sessions and herbal wellness plans are complementary services. Results vary from person to person and no specific outcome, improvement, cure or timeline is promised or guaranteed. Herbal products and plans are not intended to diagnose, treat, cure or prevent any disease.
You remain responsible for your medical care
You agree to continue working with your physician or surgeon, to follow their post-operative instructions, to obtain their clearance before hands-on bodywork, and to review any herbal plan with them — especially if you are pregnant, nursing, taking medication, or managing a diagnosed condition.
Honest disclosure
You agree to disclose all relevant health conditions, surgeries, medications, supplements and allergies before your session or plan. Services may be declined or modified at the provider's discretion for your safety.
Assumption of risk and release
You voluntarily accept all risks associated with aesthetic bodywork, compression, wrapping, contouring devices and the use of herbs, including discomfort, bruising, skin sensitivity or allergic reaction. You release TLC Self Care and its provider from any and all liability, claims or damages arising from your participation, to the fullest extent permitted by Connecticut law.
Deposits, payments and attendance
A 10% deposit via Zelle to 203-927-0247 is due at the time of booking and secures your appointment time. The $35 herbal healing plan fee is due via Zelle before your plan is written. Deposits and plan fees are non-refundable; appointments may be rescheduled with at least 24 hours notice.
Emergencies
If you experience severe pain, fever, heavy bleeding, breathing difficulty or any medical emergency, stop your session and contact your surgeon, physician or emergency services immediately.
By checking the waiver box on the booking form or the healing plan form, you electronically sign this agreement on behalf of yourself. Printed copies are available at the studio on request.
